Call me old fashioned, but I like my Window buttons to be in the standard location! If I intend to use a skin for any length of time, I generally edit the buttons so that they behave more like the standard Windows ones.
For example, I purchased the Carbonix suite. I then edited the Carbonix graphics and buttons so the minimise was back on the right instead of the left, and I even made sure the corner background "grows and shrinks" depending on how many buttons are on the window. I do this by copying the existing buttons in SkinStudio and tweaking the locations and display attributes.
Other than that, I don't tend to add things. I can cut and paste a few graphics but nothing too complicated. For instance, I had a go at adding shellstyles to the BlueCurve skin, but it all just got too hard
